I love the direction this is headed, and I think any of the options could work.

Just gonna throw out an idea that kind of blends a couple of them: what if it’s his wife, and she loved music and was just beginning to experience success when she had an accident/was diagnosed with a debilitating disease. She missed music so much that the husband decided to learn piano just for her, but kept it a secret because he wanted to surprise her; thus, she’s sad because he’s spending a lot of time away from her, and she doesn’t know why. Then, the night before he’s about to reveal his big surprise, she takes a sudden turn for the worse. Whether she dies before he has a chance to tell her/play for her would need to be decided… if not, maybe he feels that it was a waste of time to learn the piano, but then in the dream the wife encourages him to use his new-found gift to bless others.

Or, slight variant, she does know and knows that he’s doing it for her, but is still sad because she’d rather just spend time with him because she knows she doesn’t have long to live, but doesn’t have the heart to tell him that. This way he would have less to regret, because at least she knew, but he would still be regretful because he didn’t know what she really wanted… or something like that.
